Eligibility Criteria
Choosing hospice care is a personal decision that begins with understanding whether it is the right option for the patient and their family. At Lumiere Crest Hospice, we guide individuals through the eligibility process by working closely with the patient, their loved ones, and the attending physician to determine whether hospice care is appropriate.
Signs That Hospice May Be Appropriate
While every situation is unique, the following changes may indicate that it is time to consider a hospice evaluation:
- Increased fatigue or weakness.
- Shortness of breath, even while resting.
- Spending most of the day in bed or seated in a chair.
- Noticeable weight loss over the past six months.
- Frequent calls or visits to the physician due to declining health.
- Ongoing need for medication to manage physical pain.
- Multiple falls within the past six months.
- Requiring assistance with daily activities, such as bathing, dressing, eating, cooking, walking, or getting out of bed.
- A physician has indicated that life expectancy may be limited.
Experiencing several of these changes does not automatically qualify someone for hospice care, but they may indicate that an evaluation would be beneficial.
Admission Guidelines
A patient may be considered for hospice care when:
- They have been diagnosed with a life-limiting illness.
- Their attending physician determines a life expectancy of six months or less if the illness follows its expected course.
- The patient chooses a palliative, comfort-focused plan of care rather than curative treatment.
- The patient and family elect to receive hospice services.
Final eligibility is determined by the attending physician in collaboration with the Hospice Medical Director.
